In order to bill you for your donation we may collect credit card information including your card type, credit card number, and expiration date. We also collect shipping address information. This includes the name, address and phone number of the Campaign Organizer;
Information we receive from you (or that is provided to us on your behalf) on applications, survey responses and other forms you submit to us, such as your name, address, email address, telephone number;
